Osteopathic Manipulative Medicine is the study of therapeutic applications of manual techniques used to ease structural and functional issues in the bones, joints, tissues, and muscles of the body potentially providing comfort which could mean ease of pain, relaxation, and increased mobility. DOs or Doctors of Osteopathic Medicine receive extensive training in school and may continue on in residency/fellowship. On the surface these offerings can look like massage or chiropractic work. OMM lineage pulls from teachings of the body’s innate desire to self regulate and return to functionality. DOs are taught a holistic, whole person approach to wellness and health.
OMM can have similarities to Swedish massage and chiropractic work but it is not the same. Clothes also remain on the entire time.
OMM was my first non-pharmacologic intervention I would offer patients. It can be very gentle and can address a variety of conditions beyond muscle pain.
Relational Bodywork and Somatic Education (BASE) is a body orientated approach developed by one of my teachers, Dave Berger. It follows the lineage of Somatic Experiencing. Trauma, or nervous system regulation outside of the window of tolerance is felt and experienced in the organs/viscera, bones, tissues, and fascia. This is interoception which provides information about our emotional state. When a body is in coherence the various parts of the body move and feel connected signaling safety and more capacity. If the interception is signaling alarm, such as pain, discomfort, stress, then the person may enter a state of heightened arousal and activation. If this happens chronically, a self-perpetuating feedback loop can develop which over time can manifest symptom of intrapersonal challenges, relationship, work and educational problems, chronic pain, headaches, gastrointestinal problems, sleep issues, feeling detached, anxiety, PTSD and other difficulties. The body will hold psychological trauma and vice versa. Mind and body are interconnected. By providing hands on work with BASE we can both track what is happening in the body and create connection and relation. This can lead to potential interruption of the sensing loop in the body leading to expansion and ease.
OMM is about manipulating a part of the body to potentially “Fix” while BASE is supporting and encouraging relational attachments and bonding through touch leading to improved psychological and emotional wellbeing. Highly medicalized clients often do not experience touch that isn’t trying to find an answer or is trying to fix a problem. In my practice, I often blend the two disciplines, pendulating between the two.
