Initial Consultation: $600, 60 - 90 minutes. For any new patient.

Follow Up Visits With Medication Management: $400, 60 minutes.

Follow Up Visit with Somatic Experiencing, OMT, BASE, Yoga: $250, 60 minutes.

Home Visits Follow Up: Starting at $600, pricing depends on distance for travel and length of visit. Price will be discussed and set when visit confirmed.

Physician Membership: $150/month. Includes 30 minutes/month communication via text, email, phone, or virtual. Includes forms completion, prior authorizations for medications, medication refills, care coordination.

For clients with Physician Membership
 + Follow Up Visits are $200, 60 minutes. 
 + Home visits start at $300, length of time varies. pricing depends on distance for travel and length of visit. Price will be discussed and set when visit confirmed.

Care Coordination Services - non membership pricing

Phone Calls with medical decision making: $150, up to 30 minutes.

Prior Auth for Rx: $50; Prior Auth Appeal: $150.

Simple Forms: $50.

Care Coordination and Complex Forms: $150/30 minutes of work, rounds to the nearest 30 minutes.
